Assertive Community Treatment Team

Our ACTT provides comprehensive community based care to those living with mental health issues and who are dually diagnosed. The focus of our program is to provide each participant with wrap around services, using a person-centered approach. What makes our ACTT unique, is the belief and use of the principles and philosophies of recovery; believing that everyone has the right to “drive” their own care, to dream, and live a meaningful life that they have defined for themselves.

Each participant’s care is individualized and specific to meeting their needs and working on their goals for recovery. Our participants are seen beyond their diagnosis, and take on an active role in their care. We focus on working with the person in several life domains such as mental/emotional health, finances, housing, physical health, and personal safety, substance use, occupational, social, and spiritual.
